PAC Forms

"Programme as Completed"(or PAC) forms are an essential part of additional information required for any S4C production, and are needed for all programmes.

S4C is currently developing a new system for the PAC forms, whereby the production companies will be asked to enter the programme details on-line: look out for announcements on this website. In the meantime, here are the forms that will assist you to deliver all the necessary information.

All of these documents are supplied as templates for Microsoft Word documents. The best practice is to save them as templates in the "Templates" folder on your hard disk, and then use these to create new documents.

If you are using Internet Explorer on a Windows PC, you should:

  1. right-click on the name of the form you want
  2. choose "save target as"
  3. save the file as a ".dot" file in the "Templates" folder on your hard disk

You can then choose these templates to create new documents, in Microsoft Word, which will be ready to receive the programme information. As these templates contain macros, you need to click "enable macros" to be able to use the functions.

  • Programme delivery form (PAC-a)
  • This form, contains all the information needed for transmission, and the information required by advertising sales, marketing, presentation and the subtitling departments
  • CONTRIBUTORS' FORM (PAC-b1)
  • For artist payment information & Equity engagements - to be completed for single programmes or after the last episode in the series
